What to Expect at
Your First Visit

We ask patients to arrive 15 minutes early to complete important documents including:
1.) Patient Information
2.) Medical & Dental History
3.) Emergency Contact + HIPPA Policy
4.) Practice Policies
5.) Drivers License + Dental Insurance Card

We’re a fee-for-service private practice, & we are out-of-network with all dental insurance companies.
Patients are responsible for payment of the treatment rendered with or without dental insurance.
We will inform the patient of the cost of the examination including any radiographs necessary. The cost of the exam is dependent on the type of exam & the type of radiographs taken.
We can provide the patient with the information required to submit a dental claim once services are completed.

The doctor will review all of your registration forms, focusing on your medical & dental history.
The doctor will also ask you questions about any concerns and/or goals for your oral health.

Depending on the type of exam & your problem areas, different types of radiographs may be taken to gather more information related to the area.
The doctor will prescribe what radiographs are recommended based on your needs & concerns.
Intraoral photos may also be taken so we can have a reference & documentation on areas of concern.

The doctor will perform a full oral examination which includes your head & neck, teeth, gums, tongue, existing fillings & prosthetics, & other areas of concern.
The dental assistant will help document any findings that are noted during the exam.

Dr. Pham will take the time to discuss all the findings including review of the radiographs & photos that were taken.
It’s important to us that you understand the condition of your oral health. The more knowledge & information provided, the better you’re able to make a decision on your oral health.

Dr. Pham will discuss different treatment options depending on the diagnosis & your concerns. She will discuss the risks & benefits associated with those options & provide recommendations personalized to you.

For simpler cases, Dr. Pham & our office will be able to provide a comprehensive treatment plan with estimated costs associated with the services.
Financing options can be provided & overall planning of the appointments can be discussed.
For more complex cases, Dr. Pham may need additional scans or impressions in order to develop a comprehensive, staged treatment plan for you.
Another appointment may be scheduled that is dedicated to presenting you with custom plan.
There is no pressure to make a decision or accept treatment on the same day.
