Your first visit is an opportunity for us to understand your unique dental history, concerns, and goals. Our approach is thoughtful and personalized—we take the time needed to listen, evaluate, and develop recommendations that are tailored specifically to you.

What to Bring

To help us provide the best care possible, please bring the following items to your first visit:

  • A valid driver’s license or photo ID

  • A list of your current and past medications

  • Your dental insurance information (PPO only, if applicable)

  • Previous dental prosthetics (dentures, partials, night guards, etc.)

  • Information and documents related to a referring provider

  • Information and documents related to major previous medical/dental treatment

  • Your preferred pharmacy information


A Warm Welcome

As a private specialty practice, we intentionally schedule dedicated time for each patient so we can provide thoughtful evaluation and personalized attention. Some patients visit us for routine dental care, while others are referred for complex restorative, prosthodontic, or oncology-related dental needs.

Getting to Know You

Your first visit is an opportunity for us to understand your dental history, concerns, and goals. As a private specialty practice, we take the time needed to listen carefully, perform a thorough evaluation, and develop recommendations tailored specifically to your needs.

As part of this process, you’ll complete a few forms, including your medical history, dental history, and a review of our office policies. Our team is always available to help if you have any questions while filling these out.

Dr. Diana Pham will personally review your medical and dental history and take time to ask thoughtful questions about your experiences, concerns, and goals. Understanding your story helps us develop recommendations that are specific to your needs.

A Gentle Examination

Dr. Pham will perform a careful, thorough exam of your teeth and gums. This may include digital X-rays (radiographs), which are taken only as needed based, findings during the exam, and your preferences. We always explain what we’re doing and make sure you feel comfortable every step of the way.

Clear, Simple Explanations

After your exam, we’ll talk with you about what we see. As a private dental practice, we prioritize transparency and patient choice. If any treatment is needed, we’ll explain your options in clear, easy-to-understand terms—without feeling rushed or pressured. You’ll have plenty of time to ask questions so you can feel confident in your care.

Personalized Care Plan

If needed, we’ll create a care plan that fits your needs and comfort level. In many cases, we can provide treatment plan estimates the same day.

For more complex treatment needs, additional records such as photographs, digital scans, impressions, or radiographs may be collected to allow for a thorough evaluation of your anatomy, bite, and treatment options. When appropriate, we provide a dedicated second appointment for a comprehensive case presentation, where Dr. Pham will review her findings, discuss recommendations, and answer your questions.

Before You Leave

Before you go, we’ll help you schedule your next visit if needed and provide any instructions for care at home. Our team is always available if questions come up later.

Payment for Your Visit

We believe financial discussions should be clear and transparent from the beginning.

Payment for your exam is due at the time of your visit, regardless of insurance coverage. We’re happy to provide all required information and dental codes so you can submit a claim and receive reimbursement directly from your insurance.
