When you provide D PHAM DDS PLLC dba Narrative Dentistry with your mobile phone number, you agree that we may send you text messages (including SMS and MMS) to that phone number. Narrative Dentistry may send you up to 5 messages per month.
You will receive a confirmation text message, and you may need to reply as instructed to complete registration. Message and data rates apply. Reply HELP for help; Reply STOP to cancel. You agree to receive a final text message confirming your opt-out. You may opt-out at any time by texting the word STOP to +1 (713) 270-7626.
Text messages may be sent through an automatic telephone dialing system. You agree to notify Narrative Dentistry of any changes to your mobile number and update your account to reflect this change. Your carrier may prohibit or restrict certain mobile features and certain mobile features may be incompatible with your carrier or mobile device. Contact your carrier with questions regarding these issues.

What type of text messages will I receive?
Narrative Dentistry may send you the following type of text messages:
Appointment confirmations & reminders
Registration / account-related messages
Billing, balance, or insurance notices
Treatment plan or follow-up instructions
Office updates, service notices, or patient satisfaction surveys
General conversation related to treatment
Essential office communications
How do I opt-in for these text messages?

Electronically, or paper forms
Entering a phone number on a website
Text “NEW” to +1 (713) 270-7626. (Up to 5 messages a month, message and data rates may apply, and you can always text STOP to cancel.)
**No mobile opt-in message consent will be shared with third parties or affiliates for marketing purposes

Text the word STOP to +1 (713) 270-7626. After you send the SMS message STOP to us, we will send you an SMS message to confirm that you have been unsubscribed. After this, you will no longer receive SMS messages from us.
Will I be charged for the text messages I receive?
Narrative Dentistry will never charge you for the text messages you receive, however standard message and data rates may apply for any messages sent to you from us and to us from you. You will receive up to 5 messages per month. If you have any questions about your text plan or data plan, it is best to contact your wireless provider.
Important Additional Information:
There is no guarantee of an immediate response. Text messages may not be monitored 24/7. Do not use texting for urgent or emergency matters. In a medical emergency, call 911 or go to your nearest emergency room.
Privacy and Protection of Information
Any information communicated to Narrative Dentistry for text messaging and or storage will remain in the ownership of the user, which will be stored securely in accordance with our data security policy and applicable law. Narrative Dentistry will take the necessary steps to secure your personal information with safeguards appropriate to the sensitivity of the information. Narrative Dentistry will, at no time, share or redistribute this information to third parties without your consent, except where legally required to do so.
Narrative Dentistry reserves the right to change this privacy notice at its discretion. Your continued use of this service after changes to this privacy notice have been posted is taken as acceptance of those changes. It is your responsibility to monitor the privacy notice to determine whether any changes have been made. This privacy notice is subject to and applicable to all privacy laws.
